Shorter Forms, More Bookings: Fixing Dental Contact Forms
Open the contact or booking form on your clinic's website and count the fields. First name, last name, email, phone, date of birth, address, insurance, preferred dentist, preferred date, preferred time, reason for visit, how did you hear about us, message, a checkbox, a CAPTCHA.
Every one of those fields is a small reason for a patient to stop and think, "I'll do this later." And "later" often means never.
Long forms are one of the most common and easiest-to-fix problems on clinic websites.
Why every field costs you
Filling in a form is effort. On a phone, with a small keyboard and autocorrect, it's even more effort. Each extra field adds a little friction, and some patients give up partway through.
Some fields also create worry. Why do they need my date of birth? Why my home address? For people who are just curious about implant costs, a long, personal form feels like a big commitment.
The goal of the first form isn't to collect everything. It's to start a conversation. You can collect the rest later, when the patient is already booked and trusts you.
What you actually need
For a first appointment request, most clinics only need:
- Name. One field is enough. Splitting first and last name adds a field with little benefit.
- Phone number. So your team can call or message back.
- What they're interested in. A simple dropdown: checkup, implants, aligners, emergency, other.
Optional, if it really helps your team:
- Email, if you follow up by email.
- Preferred time of day, as simple choices like morning, afternoon, evening.
- A short message box, clearly marked optional.
Everything else, like date of birth, address, medical history and insurance details, can wait until the patient comes in or confirms the appointment.
Make the form easy to complete
Shortening the form is the first step. Then make the remaining fields easy:
- Use the right keyboard on mobile. Phone fields should open a number pad.
- Use dropdowns or buttons instead of free text where possible.
- Show clear labels above each field, not placeholder text that disappears when you start typing.
- Mark optional fields clearly.
- Make the button specific. "Request my appointment" or "Get my free consultation time" says more than "Submit."
- Show helpful error messages. If a phone number is missing a digit, say so clearly, and don't erase everything else the patient entered.
Reassure near the button
Right next to the submit button, add a short line that answers the patient's unspoken questions:
- What happens next? "We'll call or message you within working hours today to confirm a time."
- Is there any obligation? "No obligation. You can ask questions first."
- What about my data? "We only use your details to contact you about your appointment." Link to your privacy policy.
Small lines like these can make a real difference in whether someone clicks.
Consider a two-step form
If you really need more information, split the form into steps. Step one asks for just the treatment interest and a preferred time. Step two asks for name and phone. Once someone has started, they're more likely to finish, and the first step feels effortless.
Test it on your phone
Fill in your own form on your phone, at normal speed, as if you were a patient. Time it. Notice every moment of hesitation. Then ask a few people outside the clinic to do the same.
If you have analytics set up, check how many people start the form compared to how many submit it. A big gap tells you the form is losing people.
The form is only half the job
A great form that nobody answers is worse than no form, because the patient took action and got silence. Every submission should reach the right person instantly and get a fast, personal response. Track each one so nothing falls through the cracks, and send a quick confirmation so the patient knows their request arrived.
How we help
When we optimize landing pages for clinics, the form is one of the first things we look at. Shorter, clearer forms mean more of the people our ads bring in actually reach out. Then our follow-up system responds quickly via WhatsApp, Messenger and email, so new requests become booked consultations. Learn more about our Google Ads for dentists.
